The Nurse Practitioner applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.

Responsibilities

The SNFist NP provides intensive clinical management of high-risk enrollees and whose activities include enrollee care oversight, communication with families, significant others, PCPs, and Nursing Facility administration. Employs a variety of strategies, approaches and techniques to manage a member's health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
The SNFist NP utilizes his/her advanced nursing skills and expertise when assessing and evaluating an enrollee’s needs and requirements.

  • Collaborates with other members of an interdisciplinary care team, including the PCP and/or consulting physicians, facility administration and staff members in order to assure the member’s health, safety, and well-being in the facility.
  • Plans and implements interventions to meet care needs, and monitors and evaluates outcomes
  • Educates enrollees and/or their family regarding their care, and provides information/resources appropriate for their needs.
  • Conducts face-to-face visits and assessments of MMP enrollees in the facility.
               
Required Qualifications
  • Masters of Science in Nursing
  • Current Licensure as Advanced Practice Nurse through the Illinois Board of Nursing without restrictions.
  • 18+ months of clinical experience as a Nurse Practitioner preferably in an acute care, skilled, or long term care setting
  • Experience in managing Medicare and/or Medicaid members with chronic disease conditions including both physical and behavioral health
  • Proficiency in using of Microsoft Word, Outlook, Excel, and PowerPoint
  • Ability to use a variety of electronic information applications/software programs including electronic medical records.
  • Proficiency in analyzing and interpreting data trends.
  • Exceptional communication and interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
  • Proven ability of critical thinking, organization, written and verbal communication, problem-solving, and analytical skills.
  • This role is a part of Humana's Driver Safety program and therefore requires and individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits.
  • This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work.
  • Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required.

Preferred Qualifications
  • Medicare and/or /Medicaid Managed Care experience 
  • Adult Gerontology Nurse Practitioner (AGNP), Certified Case Manager (CCM) 
  • Skilled Nursing Facility and/or Long Term Care experience 
  • Prior experience in case management 
  • Experience with healthcare quality measures STARS, HEDIS, etc. and/or clinical program monitoring/evaluation. 
  • Bilingual 

Additional Information
  • Work at Home 75-90% local travel (regional area with field work) - MUST LIVE IN ILLINOIS
  • Monday-Friday 8-5pm; with flexible schedule as needed
